Pre-Qualifications Checklist

The Virginia State Employee Assistance Fund (VSEAF) was created exclusively for Virginia State employees who are actively employed and is experiencing financial hardship as a result of a recent, unforeseen emergency of a short-term nature.

Please complete the following Pre-Qualifications Checklist to ensure that your application adheres to the VSEAF Guidelines.

Question 1

I attest that all information I am providing on this form, and the information I will include on my application is accurate to the best of my knowledge.

Question 2

I am an Active Virginia State Employee, either full-time or part-time, who has been with the state workforce for a minimum of 1 year and consent to having this information verified during the application screening.

Question 3

I am not an employee of the Virginia Department of Corrections (VADOC) or the Virginia Commonwealth University (VCU) Health Systems which have their own employee assistance fund. Note: Academic VCU employees and UVA Medical employees are still eligible.

Question 4

The short-term, unexpected emergency occurred within the past 90 days.

Question 5

The short-term, unexpected emergency is not related to:

An immediate family member that does not live in the residence with me.
A bill addressed to someone other than the applicant.
An emergency related to loss of income or loss of employment, such as Unpaid Leave of Absence.
Unpaid monthly bills related to lack of funds, or requests based on the rising cost of gas for vehicle or home heating use.
A long-term medical situation.
A situation based on medical costs that are covered by insurance or Workers’ Compensation.
A refundable deposit or delayed compensation.
Requests related to estimates for medical care, such as an estimate for dental work or medical procedures that has not occurred.
Requests related to normal wear and tear and routine maintenance on a vehicle.
Requests for legal fees, bail, bankruptcy filing, and similar transactions.
Requests for loss of household income due to a non-lease holder or mortgage holder person moving out of the home or losing their source of income.
Tuition or educational costs for any member of the household or extended family.
Requests to supplement income due to lack of seasonal work, or between seasons - this includes teaching schedules, and delivery types of industries.

Question 6

Question 8

ድጎማ ከተሰጠኝ ድጎማው የሚከፈለው በቀጥታ ለኩባንያ ወይም ለአበዳሪ እንጂ እንደ አመልካች ለእኔ ሳይሆን እንደ ማካካሻ ጭምር መሆኑን አውቃለሁ። VSEAF DOE እርዳታዎችን በቀጥታ ለአመልካቾች የመስጠት ችሎታ የለውም።
